Sullivan welfare fraud arrests

Liberty, NY – The Sullivan County Welfare Fraud Task Force has made the following arrests: 

May 14, 2018 – Richard Vogel, 25, of Liberty was arrested for violating the conditions of his parole. After a member of the task force observed Vogel purchasing alcohol in violation of his parole conditions, a joint investigation with NYS Parole was initiated and resulted in his arrest. Vogel is currently in Sullivan County Jail awaiting a parole hearing.

May 15, 2018 – Philip A. Jacobs, 29, of Monticello was arrested and charged with three Felony counts of Filing a False Instrument in the 1st and three Misdemeanor counts of Theft of Service. It is alleged that Jacobs, a parolee, stole $143.42 in public-funded transportation by using Medicaid-funded medical transportation for transportation to and from his parole meetings in South Fallsburg. Jacobs was arraigned in the Town of Liberty Court and remanded to Sullivan County Jail on $7,500 bail.

May 17, 2018 – Michael L. Weaver, 47, of Monticello was arrested and charged with one Felony count of Filing a False Instrument in the 1st degree and one Misdemeanor count of Theft of Service. It is alleged that Weaver, a parolee, stole $12 in public-funded transportation by using Medicaid-funded medical transportation for transportation to the Department of Family Services in Liberty after he was told the day before that he could not use medical transportation for this purpose. Weaver was arraigned in the Town of Liberty Court and remanded to the Sullivan County Jail.

May 30, 2018 – Paulette Williams, 44, of Hurleyville was arrested and charged with the Class E Felony of Filing a False Instrument in the 1st degree and the Class A Misdemeanor of Theft of Service. It is alleged that Williams stole $1,286.24 in public-funded transportation by using Medicaid-funded medical transportation for non-medical purposes. Williams was issued an appearance ticket and released pending a future court date in the Town of Liberty.

May 30, 2018 – Joseph Barone, 43, of Liberty was arrested and charged with the Class E Felony of Filing a False Instrument in the 1st degree and the Class A Misdemeanor of Theft of Service. It is alleged that Barone stole $72 in public-funded transportation by using Medicaid-funded medical transportation for the non-medical purpose of transportation from his residence to Community Services in Liberty.

June 13, 2018 – Kaitlyn Hoag, 22, of Liberty was arrested and charged with the Class D Felony of Possession of A Forged Instrument 2nd, Class E Felony of Offering a False Instrument for Filing 1st, and Class B Misdemeanor of Attempted Welfare Fraud 5th. It is alleged that Hoag submitted a false shelter verification form and a false SNAP (food stamps) recertification form to the Sullivan County Department of Family Services in an attempt to obtain benefits that she was not entitled to. Hoag was arraigned in the Town of Liberty Court and remanded to the Sullivan County Jail in lieu of $2,000 bail.

June 26, 2018 – Lisa L. Kazamarick, 42, of Liberty was arrested and charged with the Class D Felony of Possession of A Forged Instrument 2nd and the Class B Misdemeanor of Attempted Welfare Fraud 5th. It is alleged that Kazamarick provided the Sullivan County Department of Family Services with a forged shelter verification form in an attempt to obtain benefits that she was not entitled to. Kazamarick was arraigned in the Town of Liberty Court and released pending a future court date.
